Girder fork head bearings coming loose

The steering bearings on my '34 Inter seem to be forever coming loose. I slacken off the pinch bolt (it's more cotter pin than pinch bolt I guess), tighten the top nut to take out the play and tighten the pinch bolt back up again. I did 400 miles on it last weekend and there was loads of play, having tightened it up before setting off.

To start with I was putting it down to everything being newly assembled so just settling down. But I've done about 2000 miles on it now so was wondering if anyone else has had this problem and what they have done to cure it. It's a bit of a pain tightning it back up again as the Andre damper has to come off first so I'd like to be able to set it right and it stay about right for longer than a few hundred miles!

Any ideas?

I had the same problem on my girder 16H.

the top nut had cracked. I'm pretty sure I got the replacent from Russles
